If you live in the metro area, you can stop by one of our offices for a health and well being consultation. However, don’t worry if you don’t live near us, you can call and we will set up an appointment to have one of nurses come to you.

Our Locations


The AFI Hospice team believes patients need to be enabled to preside over their own dying. The AFI Hospice team will center their focus on patient's priorities and serve their needs. Knowing that freedom and dignity can be impaired by severe physical and emotional distress, AFI Hospice will seek to relieve pain and suffering in the physical, emotional, social, and spiritual realms. AFI Hospice is committed to patient-centered care, professional and timely management of symptoms and comfort. AFI Hospice will be that advocate for patients who choose a peaceful, dignified end of life surrounded by family and loved ones.

 We also offer medications, equipment and supplies that are related to the hospice diagnosis.

HomeCare & Hospice has administrative offices located in Brigham City and Layton city: We serve Box Elder County, Weber County, Davis County, Salt Lake County  and Utah County.

What we provide as a hospice company:

Our office hours are Monday through Friday from 8:30 a.m. to 5:00 p.m. Our nursing staff is available 24 hours a day, seven days a week.

Our licensed home care program offers skilled nurses and home health aides to provide a variety of medical and personal care needs to patients in Box Elder, Weber, Davis and Salt Lake counties.


Our licensed home care program allows patients with short-term needs or chronic conditions to remain in their own homes. We provide services for the frail elderly, developmentally disabled, patients recovering from surgery or illness, as well as those living with long-term disability or illness.
